Preparing for your visit

Thank you for choosing ENT of Georgia for your medical care. We constantly strive to make the patient experience as streamlined and convenient as possible. To prepare for your upcoming appointment follow the 3 steps below:

Step 1:

To complete the 3 forms below, open them online, fill them in on your computer, print them, sign the printed form and bring them with you to your appointment::

• Privacy Acknowledgement
• Patient Communication
• Insurance Coverage and Referral Waiver.

Step 2:

Use our Secure Medical Patient Portal to complete your paperwork. Your background information and medical history will be entered directly into your electronic medical record. In the future you can use your Patient Portal user ID and password to request future appointments, request prescription refills and communicate with your physician / staff.

If you do not wish to enter your information online, you can complete and print these paper forms and bring them with you to your visit.
